Glucose Testing
Diabetes is a condition where your body has trouble managing the sugar in your blood. Normally, a hormone called insulin helps your body use sugar for energy. But in diabetes, either your body doesn't make enough insulin or it can't use the insulin it makes very well. This causes sugar to build up in your blood, which can lead to health problems if not controlled. People with diabetes often need to watch what they eat, stay active, and sometimes take medication or insulin to keep their blood sugar levels in check.

How do I apply for POYC with Rotunda Pharmacy?Once you've gathered all your necessary documents (including Schedule II/V forms), make photocopies of them along with your ID card and a prescription listing all your medications. Take these copies to your chosen pharmacy and request a pharmacy registration form. Complete Section A of the form and return all documents to the pharmacist. They'll fill in Section B and send everything to the Pharmacy of Your Choice Unit. The doctor will fill in Section C. Processing typically takes 7-10 days, after which you can collect your medication.

Can I change from another pharmacy to Rotunda Pharmacy?Certainly! If you're currently registered with another pharmacy but want to switch to a different one, simply fill out the change in pharmacy form. Submit the completed form to the managing pharmacist at your chosen pharmacy. They will then forward the document to the POYC unit to transfer your registration.
