Please provide a contact number. Mobile numbers are preferred as POYC may send SMS reminders for expiring permits, but if you don't have a mobile, a landline is acceptable.

A person of trust is someone we can contact if you are unavailable or unwell in case of emergency. We will need the person's ID CARD NUMBER as well as their respective mobile number. Only one person of trust can be appointed.

We will go through your Schedule IIs (Pink Card), Schedule Vs(Yellow Card) and permits so we can advise you as to whether these need to be taken care of. We will advise on the best ways to go about it if difficulties arise.

We will go through your POYC medications.

- What medications do you take and how many per day?

- Do you know what the medications are for?

- Do you feel any side effects?

Are there any medications or supplements that you need to purchase on a monthly basis which are not provided for free by POYC?

This information is needed so that healthcare professionals are made aware of any other medications or supplements that you take. Furthermore the pharmacist can assess if there might be any interactions.

When an MUR is conducted you will receive an SMS by POYC telling you that Rotunda Pharmacy has performed an MUR.
